There are cases where it is mandatory to extract (pull) the wisdom tooth.

These situations include a cracked tooth, caries of the wisdom tooth root, position of the wisdom tooth where it damages the adjacent seventh tooth, acute inflammation (e.g. abscess).

There are also cases where extraction is not mandatory.

These are:

Inflammation of the gums around the wisdom tooth,

a position at which it is not possible to clean the wisdom tooth properly,

trauma to the cheek due to the tooth,

orthodontic treatment.
